Transitioning from long-term care to home care: factors for success

Investing in comprehensive and accessible healthcare services for older adults is essential for reducing the burden on acute care facilities, promoting healthy aging, and ensuring older adults receive timely and appropriate care in the most suitable settings. Transitional care programs are one essential healthcare resource providing rehabilitative support for older adults moving from hospital to home. Transitional care programs aim to improve the health, independence, and well-being of older adults, reduce acute care costs, and ensure that older adults can age in place with dignity and quality of life in their later years.


Older adults are often challenged with accessing appropriate home care services and rehabilitative support during transitions from hospital to home, potentially leading to premature entry into long-term care facilities. Significant gaps in home care services, with services often sought too late, hinder the ability of family caregivers to provide adequate support and lead to caregiver burnout.
